In its effort to streamline procedures and to make it more convenient for its members, the Social Security System has consolidated its contribution payment forms for individual payors and employers, its press release said yesterday.
Starting Sept. 1, 2014, voluntary, self-employed, OFWs and non-working spouses will be required to use the new consolidated contribution payment form, it said.
Employers will continue to use the SSS Form R5 in the payment of their employees' contributions until the current stock of the form is consumed. Employers are required to start using the new contribution payment form on 01 November 2015, it added.
For clarifications on the new consolidated contribution form and schedule of their usage, members can inquire through the following: 24/5 SSS Call Center at 920-6446 to 55; and My SSS at www.sss.gov.ph., the press release said.*
